There are many questions to be answered about how this oil spill in the USA happened. What policy and procedures were in place to ensure an environmental catastrophe would be avoided if something were to go wrong?

How does something like this happen? As the blame game goes on I wonder how many other oil rigs do not have appropriate mechanisms to avoid another environmental mess. It will take a long time to see what bigger impacts this disaster has had on the environment, the ocean, the people and the wildlife.
